Abscess due to Staphylococcus Aureus Infection

History

  • 45 years old man
  • Good past health
  • Right abdominal wall abscess for one week
  • Treated with oral ampicillin and cloxacillin
  • No improvement with antibiotics
  • Increasing pain, pustular discharge noted from abscess
  • BP 135/69mmHg P65, temp 36°C

Progresschart

  • Incision and drainage of abscess performed at A&E
  • Continued on oral ampicillin and cloxacillin
  • Culture revealed Staphylococcus aureus (methicillin resistant)
  • Diagnosed to have community acquired methicillin resistant Staphylococcus aureus (CA-MRSA) infection
  • PCR positive for Panton-Valentine leucocidin (PVL)
  • Admitted and given one week of intravenous vancomycin
  • Subsequent history revealed recent body massage in China (? Source of CA-MRSA)
